The Significance of Level of CA-125 Antigen in Serum and Peritoneal Fluid in Patients with Endometriosis


Abstract
CA -125, a cell surface antigen was meaured with the radioimmunometric assay in serum and peritoneal fluid from one hundred and five patients with endomentriosis and ten control wemen, to determine its clinical usefulness in the diagnosis,
follo-up
and
management of endometriosis.
@ES The results were as follows:
@EN 1. The mean levels of serum CA-125 were 15.6*1.17 in normal controls and 20.3*3.29, 25.3*3.35, 34.5*6.69, 56.1*17.54u/ml in endometriotic patients with AFS(Americal Fertility Society) stage I. II, III, IV, The frequency of elevated
levels(>35u/ml)
and mean values were significantly higher in stage III/IV than those in stage I/II and normal controls. Serum CA-125 levels were positively correlated with disease severity(r=0.33, P=0.04).
2. the peritoneal fluid CA-125 levels were 149.1*21.86 in normal control women and 111.1*12.3, 134.0*11.8, 160.6*21.8 and 179.8*25.4u/ml in stage I, II, III and IV ondometriotic patients respectively. No significant differences in peritonea
fluid
CA-125 levels were found between normal controls and endometriotic patients. The peritoneal fluid CA-125 levels were not correlated with disease severity(r=0.21, P=0.07).
3. The ratios of peritoneal fluid CA-125/serum CA-125 were lower in the advanced endometritic patients, but no statisticaly significant differences were found between different stages.
4. The mean peritonealvolume increased with disease severity but there were no statistically significant differences between different stages. Peritoneal fluid volumes were positively correlated with menstrual cycle dates. The peritoneal fluid
volumes
in infertile women with endometriosis were significantly elevated than tose in fertile women with endometriosis.
5. Serum CA-125 levels wer significantly lowered after hormonal treatment, such as Danazol, Decapeptyl and Nafarelin.
6. The infertility incidence was 31.4% in patients with endometriosis. The crude pregnancy rate was 48.4% in infertile patients. The mean interval to achieve pregnancies following hormonal treatment was 5.6 months.
7. The serum CA-125 levels were well correlated in 86% study group patients in terms of AFS endometriotic scores.
In conclusion, the serum CA-125 measurement is considered to be valuable in diagnosis and follow-up of endometriotic patients with stage III/IV disease.
